Veterinarian, Veterinary Technician, and Unlicensed Practice Complaint If you have a concern that a veterinarian, veterinary technician, or a person practicing veterinary medicine without a license in the state of Wisconsin, is in violation of the state’s law relating to the veterinary field, you may file a complaint by completing the following online complaint form.

This complaint and the information provided may be used in efforts to resolve the problem and may be shared with the party complained against. It may also be used to enforce applicable state laws. Under Wisconsin's Open Records Law, Wis. Stats. § 19.21, et seq., this complaint will be available for public review upon request unless an investigation is pending or nondisclosure is authorized by law. Omit or mark out any confidential or personal information (e.g., checking account number, credit card number, Social Security Number, date of birth, etc.) prior to submitting information to our office.

The department may refer complaints outside the department’s jurisdiction to local authorities or another state.
