Dairy Plant Field Representatives
Are you looking to obtain a new Milk Producer License for a producer who intends to ship milk to your dairy plant? You are in the right place!
To apply for a new Milk Producer License, click “Start New Application”. Do NOT log on. This will walk you through the new license process step-by-step. Be prepared to upload the following completed documents: License Application Affirmation form signed by the milk producer, Water Supply Report and negative water results sampled within 2 years for each well, Field Rep Farm Inspection, Notarized Premise ID Exemption if applicable, Notarized SSN Exemption if applicable and the DNR NPSV approval letter if applicable.
Once the application is submitted, the field rep must contact the sanitarian to schedule the licensing inspection. If the sanitarian determines that an inspection cannot be scheduled prior to the first pick up and a temporary license is needed, contact the Eau Claire office staff or the general line at (715) 839-3844.
This license is not transferable between applicants or facility locations
